Instructions:
A copy of your current certification card is required to complete registration for all AHA Renewal Courses. When registering for a renewal course, select the "replacement card" option to have a new card generated. If you have lost your card and need a replacement, please follow the directions below to start the process or call 513.636.6992 for assistance.

Certification Card Replacement

The American Heart Association (AHA) requires copies of certification cards for all renewal courses. Cincinnati Children's Center for Simulation and Research offers the following AHA courses:

Pediatric Advanced Life Support (PALS)
Advanced Cardiac Life Support (ACLS)
Basic Life Support (BLS)

Associated Fees
Certification Card Replacement...........$25.00 per card.
