  Early bird registration of $285 ends on April 6, 2012. Registration fee after April 6 will be $320.
  
  Registration includes access to all sessions and talks, a conference tote bag with Program & Abstracts and related materials, wireless internet access at the venue, refreshment breaks, a conference "icebreaker" reception, and vendor lunch (for first 75 registrants).

  SPNHC Student annual membership is only $20 and is available online at www.spnhc.org.

4. One Day Registration: $150.00 Details
  
  One day registration includes access to all sessions and talks for one day, a conference tote bag with Program & Abstracts and related materials, wireless internet access at the venue, and refreshment breaks. You are welcome to register for paid events that occur on the day you will attend and any field trips and workshops.

Payment of registration fees, field trips, workshops and banquet is due upon receipt of this form. On-line registration must be paid with a credit card. To pay by check, print off your completed registration form and mail your check or money order made payable to Yale University, write SPNHC 2012 in the memo area, and mail to:

All prices are in US funds and must be paid with US currency.
