REGISTRATION INSTRUCTIONS


Register and make your hotel reservations online—quick and easy!
1.    Complete online registration form and submit payment via VISA, MasterCard, Discover, American Express, check or money order.  Confirmation will be sent via email.
2.    CONFERENCE HOTEL reservations are included in your registration process for the conference room block and special rate.  Hotel guests registered in the conference room block are eligible for discounts.  

On-Site Registration begins at 5:00 PM, July 8, 2013.

Group Discount Available
(Available for Summer Institute Only)

Group rates are available for four or more participants representing the same organization.

  • Four (4) or more participants from the same organization/agency will receive a 5% discount per person on registration fees.
  • Each member of a group must register at the same time under one registration and specify the group name. 
  • A $25.00 fee will be added to your registration if you cancel your individual registration and change it to a group registration.

NOTE: You must register the group under one registration to receive the group discount.  Individual registrants will not receive the group discount and will be charged a $25.00 fee to cancel and add into a group registration. 

You may take advantage of only one discount per registration.  If you choose to take the group discount then the discount for staying at the hotel will not apply.

Qualifying for Student Registration

  • Students are requested to document full-time enrollment to be eligible for discounted conference registration.  Documentation is requested in the form of a letter on your school's letterhead co-signed by student and advisor to be uploaded during the registration process.  Please include the following in the letter: home school, major, level of program (e.g., graduate), current course load, stage in academic career, school advisor's name, title, and contact information.

To Register as a Student
  • The student's registration will be confirmed upon receipt of payment and documentation letter (see previous section on Qualifying for Student Registration).
  • If documentation is not received, the individual rate will apply.

Registration Confirmation

If your registration is submitted by June 24, 2013 confirmation of registration will be emailed to you once actual payment has been received.  For those that registered on or before June 24, 2013 and did not receive a confirmation, please email inquiries to HallKM@uthscsa.edu.  If you do not register by June 24, 2013, contact 210-567-1480 to inquire about space availability for the conference.  You are responsible for ensuring that your registration has been received.

Refunds & Cancellation Policy

UT Health Science Center Academic Center for Evidence-Based Practice reserves the right to limit enrollment or to cancel any course no less than one week prior to the activity.  Should circumstances make this necessary, only registered attendees will be informed.  In this event, tuition would be refunded in full.

You are encouraged to send a substitute if you are unable to attend, once registered.  Cancellations must be received in writing and postmarked or emailed no later than June 24, 2013 at 5:00 PM CST.  The registration fee will be refunded, minus a $100 administrative fee if notice is received by June 24, 2013.  After June 24, 2013 there will be no refund of registration fees.  Registration fees will be refunded after the conference.  Registration fees will be refunded after the conference.

The conference hosts reserve the right to substitute speakers or cancel and reschedule sessions.

    Payment Deadlines are:
    Early Registration:
    Payment must be postmarked by May 20, 2013
    Regular Registration: Payment must be postmarked by June 24, 2013

    Anything postmarked after these dates will be subject to the current registration fee according to the postmarked date.

    Registration requires full payment, otherwise your registration will not be valid.
