Times:
Leave: 1:00pm Sunday, June 4 from FPC West parking lot
Pick-up: 3:30pm Friday, June 9 at FPC West parking lot

Cost of Trip: $440 
Payable by personal check or FPC Grocery credits. Contact Karen at kcobb@fpcboulder.org if you need to set up a payment plan.

Required Forms:
Habitat For Humanity Waiver - print, complete, and drop off or mail to:
First Pres Boulder, Attn: Karen Cobb, 1820 15th St, Boulder, CO 80302
Packing List (for you to keep)
Parent Packet (for you to keep)

Transportation:
Depending on the number of participants, transportation will be provided by church vans and other vehicles as needed.

Important Dates:
Sunday, April 30 - Parent Information Meeting & Online Registration Begins
Sunday, May 7 - Commissioning of the Mission Trip students at FPC during 9:15am AND 10:45am services, please arrive 9:00am. (Mandatory)
Friday, May 26 - Final day to register, pay and complete paperwork

    Make personal check for $490 (including late fee)  payable to "First Pres Boulder" and drop off or mail by May 26th to:

    Please DO NOT give checks to leaders, interns, or Kyle at check-in on Sunday as they need to focus on the kids and the trip. Please either mail your payment or drop off to the church office, thank you!

    Please complete the Grocery Transfer Form or contact Pat Burgener at 303-494-4057 or pat@business-computers.com to request your funds for the Alamosa Mission Trip.

    - Credit Card:  submit online - price includes 4.95% credit card fee
