The goal of this program is to provide comprehensive education on topics that are of significant importance to providers caring for geriatric patients. 

Program Objectives:  Following completion of this program, attendees will be able to:

  • Evaluate fall risk and make recommendations to improve patient safety
  • Discuss the four-part approach to management of person's and their family with dementia
  • Effectively coordinate patient care when transferring from the hospital to reduce readmissions
  • Use tools and methodologies for communicating with patients and their families at the end of life. 

Target Audience: Providers, nurses, social workers and case managers practicing in Primary Care, Geriatrics and related specialties

Continuing Education:

  • CME Credit: Providence Portland Medical Center is accredited by the Oregon Medical Association to sponsor continuing medical education for physicians.  Application pending.
  • AAFP Prescribed Credit:  This Live activity is pending review and acceptance for prescribed credit by the American Academy of Family Physicians.
  • RN CEU Credit:  Providence St. Vincent Medical Center Department of Nursing is a provider approved by the California Board of Registered Nursing (Provider #CEP8693). Application pending.
