2015 March of Dimes Conference for Health Professionals

 Daily (RN)* $150
 Full Conference (MD) $360
 Daily (MD) $190 
 Late Registration (paid on or after 03/04/15)
 Full Conference (RN)* $410
 Daily (RN)* $220 
 Full Conference (MD) $460
 Daily (MD) $245 
*MFT/LCSW, physicians in training, retired persons, all students and others pay the RN fee.
REGISTRATION FEE INCLUDES:
           Continental breakfast, break refreshments, lunch and continuing education certification (if applicable).

 
REGISTRATION PAYMENT
REGISTRATIONS ARE ONLY ACCEPTED
BY MAIL OR ONLINE

Registration payment is by check (payable to 'March of  Dimes') or credit card (American Express, Discover, MasterCard  or Visa) ONLY.
All outstanding registration fees must be paid in full by 03/11/15.
Unpaid registrations are not guaranteed and will be subject to a late payment fee of $25.
Please read the 'Terms and Conditions' section below for important information on refunds for canceled registrations.


GROUP DISCOUNTS
Groups of 4 or more receive a 10% discount on registration fees. All registrations must be received together. If your group contains different registration types (e.g. daily AND full conference), you must register by mail to receive the discount.
Mail Registration: Complete one registration form for each group member and mail with payment to the address above.
Online Registration: Complete the registration form for the first group member, then click ‘Add Another Person’ to create a group and add members. You must provide a different email address for each group member so that all group members receive a confirmation email. The discount for all group members will be calculated automatically at checkout.

CONFERENCE MATERIALS
Speaker-approved presentations will be available to download in pdf format at marchofdimes.org/ca/2015conference no later than one week after the close of the conference.  Please note that some presentations are omitted at the request of the speaker.

TERMS AND CONDITIONS
Registration terms and conditions are listed below. 
Photography
By attending this event, you give March  of Dimes permission to photograph you at the event (limited to general crowd/background shots) and to use photos on our website, in print and  in other media for March of Dimes promotion. If you do not wish to have your photograph used, please contact our on-site event staff.
Payment
Registrations that are not paid in full by 03/11/15 are not guaranteed and will be subject to a late  payment fee of $25.
Cancellations and Refunds
Cancellations must be received no later than 03/06/15 to receive a refund (less non-refundable processing fee of $50). Cancellations after this date will not be eligible for a refund.  Refunds for canceled registrations will be received by the original payment method.   
Credit Card Refunds: Refunds can only be made to the credit card used in the original purchase.  
Check Refunds: Please allow 2-3 weeks for  processing of check refunds.
Substitute attendees are permitted. If you are making a substitution,  please include the name and contact information (including the email address) of  the person that you wish to substitute.   The deadline to submit changes to your registration is 2pm on 03/11/15.
