COMMUNITIES ARE RESPONDING EVERYDAY (C.A.R.E)

The National Association of Free Clinics (NAFC) will be sponsoring a one day free medical clinic for the uninsured on Monday, August 29, 2011 in New Orleans, Louisiana at the Ernest N. Morial Convention Center.

The New Orleans C.A.R.E. Clinic will offer free basic medical care and health education.   Being a volunteer at this event will give you the opportunity to help hundreds of people gain access to medical care they otherwise may not receive.

The NAFC C.A.R.E. clinics are made possible through generous donations.  If you would like to donate you can:

PATIENT INFORMATION
:

The New Orleans C.A.R.E. Clinic patient hours are from 11:00am to 7:00pm.  

Patients are encouraged to call ahead and make an appointment for this clinic event.  Patients can call 877-236-7617 to make an appointment. 

Uninsured Patients Only. Walk-ins will be seen on a limited, first-come first-serve basis.

Please note:

- This event is for those WITHOUT insurance.  Recipients of Medicare, Medicaid, or Veterans Administration benefits are not eligible.
- If you are taking medications, please bring your medicine bottles.
- Parking will be free.

The shift times for Medical and Non-Medical Volunteers are:


Saturday, August 27th - Set-up - FULL
First Shift        1:00pm - 5:00pm

Sunday, August 28th - Set-up - FULL
All Day           9:00am - 5:00pm
First Shift        9:00am - 1:00pm
Second Shift   1:00pm - 5:00pm 


Monday, August 29th - Clinic

All Day           9:00am - 9:00pm
First Shift       9:00am - 4:00pm
Second Shift   3:00pm - 9:00pm 
Third Shift (For MDs, DOs, FNPs, APRNs, PAs ONLY) 5:00pm - 9:00pm

Each person volunteering for the C.A.R.E. Clinic must be registered in order to have a name badge and job assignment for the event.  If you are registering as a group, a registration must be done for each person.



OUT OF STATE PRACTITIONER VOLUNTEERS - Please see the documents below that you need to fill out and return to the Louisiana State Board of Medical Examiners either via email or fax.  Please let us know when you have submitted the information to the Board so we may also send a letter from the NAFC supporting your request.  Please fax a copy of your license to 866-875-3827 or you may scan and email it to care@freeclinics.us

OUT OF STATE NURSES (RN)
- As a professional courtesy the Louisiana State Board of Nursing (LSBN) does not require nurses providing care in the state for a brief period of time to be licensed in the state they do however require all out of state nurses to apply for a temporary license.  Please download a copy of the attached form, fill it out and fax the completed form, your license, and drivers license to 225-755-7581.  Please also fax your license to 866-875-3827 or you may scan and email to
care@freeclinics.us

Please check the LSBN website approximately 48 hours after faxing in your application for approval at 
www.lsbn.state.la.us,
to ensure you have been granted permission to practice.

At this time Louisiana does not allow out of state Nurse Practitioners to practice but NPs from OUT OF STATE may serve as a nurse on the clinic day.


MEDICAL PROFESSIONALS: THE NAFC HAS OBTAINED MALPRACTICE COVERAGE FOR THIS EVENT.
