Connecting care across the continuum and connecting with patients and consumers has never been more relevant to health systems than today. Risk contracts and bundled payments, and the explosion of mobile technologies and remote care capabilities collide with strong incentives for health and wellness. Interoperability also plays a role in realizing the benefits of connected care and health. Join us in Austin to share and discuss strategies, tactics and results with health-system and payer executives leading these new breakthroughs in health and care.


WHO SHOULD ATTEND

This Forum is designed for SI member executives who are interested in integrating communities and consumers, population health and related data strategies, leveraging personal, mobile and retail healthcare trends, and assuming financial risk for patients and populations and creating insurance options.
