Registration Fees:

First time optometrist registration = $399.00 per event*

All additional optometrist or staff member registrations = $199.00 per event 
Post graduate optometrists with 5 years or less in practice = $199.00 per event 

*In conformance with PhRMA guidelines, the program fees are intended to balance the transfer of value. 

The reduced registration fee of $199.00 for additional optometrists or staff members is applied only if the additional registered optometrists or staff members work in the same practice as the first optometrist, or for post graduates with 5 years or less in practice. 

Once your registration has been submitted, your registration record will be reviewed. Once confirmed, your credit card will be charged within 1 week of registration.

Travel Arrangements:
Any travel (airfare, driving, etc), hotel, and parking arrangements will not be covered and/or reimbursed by Allergan. The only meal that will be provided by Allergan at the event is a lunch and beverages.


Cancellation Policy:
If you are no longer able to attend the event you register for, a cancellation request must be submitted in writing via email to [Insert email address link]. 

If your cancellation request is submitted prior to 2 weeks before an event date, you will receive a full refund of the registration fee(s) that were processed.  

If your registration is cancelled within 2 weeks of the program or you do not attend the day of the event, you will receive a 50% refund of the registration fee(s) that were processed.
