Group Registration Policy

Group registration is available for four or more participants representing the same organization.

    Registrations for four (4) or more participants from the same organization/agency
    will require payment in full at the time of registration.
    Note: 50% of this amount will be considered a non-refundable deposit.

    Each member of a group must register at the same time under the same
    registration and specify the group name.

Registration Confirmation

If your registration is submitted by April 22, 2013 confirmation of registration will be emailed to you once actual payment has been received. For those that registered on or before April 22, 2013 and did not receive a confirmation, please email inquiries to lunat3@uthscsa.edu. If you do not register by April 22, 2013, contact 210-567-5845 to inquire about space availability for the workshop.  You are responsible for ensuring that your registration has been received.

Refunds & Cancellation Policy

UT Health Science Center Academic Center for Evidence-Based Practice reserves the right to limit enrollment or to cancel any course no less than one week prior to the activity.  Should circumstances make this necessary, only registered attendees will be informed.  In this event, tuition would be refunded in full.

Individual Registration
You are encouraged to send a substitute if you are unable to attend, once registered. 
Cancellations must be received in writing and postmarked or emailed no later than
April 22, 2013 at 5:00 PM CST.  The registration fee will be refunded, minus a $100 administrative fee if notice is received by April 22, 2013.  After April 22, 2013 there will be no refund of registration fees.  Registration fees will be refunded after the workshop.

Group Registration
A group is encouraged to send a substitute if an individual is unable to attend.  If cancellation is necessary, 50% of the registration fee will be refunded minus a $100 administrative fee for each cancelled registration.  Cancellation request must be received in writing and postmarked or emailed no later than April 22, 2013 at
5:00 PM CST. After April 22, 2013 there will be no refund of registration fees.  Registration fees will be refunded after the workshop.

The conference hosts reserve the right to substitute speakers or cancel and reschedule sessions.
