The Connecticut C.A.R.E Clinic will take place at the Connecticut Convention Center February 3, 2010  12:00 p.m.-7:00 p.m.

Our commitment is to all of the patients and the last patient may not leave the building until 8 p.m. Therefore volunteer shifts run until 8:00 p.m.

The deadline for Volunteer Registration is February 1, 2010

VOLUNTEER GUIDELINES

ORIENTATION will be held on Wednesday February 3rd  at the Connecticut Convention Center.  Orientation will be held in group sessions during the first 30 minutes of your shift therefore prompt arrival is necessary.
Orientation is mandatory.

Pre-registration for volunteers is strongly advised.  We cannot guarantee a volunteer opportunity as either a health care professional or support person if you have not preregistered.

Volunteers cannot be seen as patients.

You are required to CHECK IN at the volunteer registration upon arrival.  


Verification of community service hours worked will be provided upon request.

T-Shirts and name badges will be provided to all volunteers; these must be worn during the event. 

Food and drinks will be provided for all volunteers. 


Casual, modest attire is recommended (scrubs, capris, etc.) Comfortable shoes are a must. 
Please no short/belly tops, strapless tops/dress or short tops with bare midriffs. 

No smoking or pets at this event


Parking and directions information will be provided when you receive your follow-up communication from NAFC

FOR CLINICAL VOLUNTEERS ONLY

OUT OF STATE MEDICAL VOLUNTEERS-IF YOU ARE NOT A LICENSED PROVIDER IN THE STATE OF CONNECTICUT, PLEASE CONTACT THE CONNECTICUT MEDICAL BOARD FOR LICENSURE INFORMATION.

Licensed Professionals (M.D.,R.N., N.P., P.A. etc.) Bring proof of your professional licensure if you have not preregistered. It is recommended you fax your a copy of your licensure or your license to 866-875-3827 or you may scan and email to ctclinc@freeclinics.us

Bring your own diagnostic equipment (such as otoscope/ophthalmoscope, phoropter, stethoscope, blood pressure cuff, etc) as these instruments are available only in limited quantity. If you have a favorite instrument, please bring it with you.

Basic supplies will be available (gloves, hand sanitizer, etc). 


If you have prescriptive authority, please bring your own tamper-proof prescription pads.
